Admission / Status
Evaluate the clinical information available when the status decision was made and the expected course of medically necessary hospital care.
UM Physician Advisors LLC provides physician advisor services for hospitals and health systems across patient status, medical necessity, continued stay, retrospective review, appeals, and complex utilization-management cases. Hospitals need physician-advisor support at the points where admission status, continued stay, clinical change, payer scrutiny, discharge barriers, and post-acute transition intersect. Our review model connects the governing framework to the complete documented clinical trajectory.
Evaluate the clinical information available when the status decision was made and the expected course of medically necessary hospital care.
Determine whether evolving severity, treatment intensity, complications, response, and continued needs still support acute hospitalization.
Recognize when unexpected improvement, deterioration, procedures, or new complications appropriately change the level-of-care analysis.
Separate ongoing acute medical necessity from post-acute needs, placement barriers, administrative delay, and discharge readiness.
Engagements can support a single workflow or extend across patient status, concurrent review, disputes, complex cases, and transitions of care.
Physician review of inpatient versus observation questions, conversion decisions, and the clinical circumstances supporting the requested status.
Explore review types →Day-specific review of evolving severity, active treatment, response, complications, continued acute needs, and transition readiness.
Explore review types →When applicable, reconstruct the contemporaneous admission expectation and distinguish it from later clinical developments and continued-stay questions.
Review the framework →Verify current procedure classification and apply the appropriate Medicare patient-status pathway rather than relying on historical assumptions.
Review procedure status →Distinguish medically necessary acute hospitalization from post-acute placement, administrative delay, family preference, or other non-acute barriers.
Explore review types →Independent reassessment of disputed determinations using the complete record, governing framework, and concise physician rationale.
Explore review types →Review IRF, SNF, LTACH, NOMNC, rehabilitation, skilled needs, functional trajectory, and readiness for the next level of care.
See case examples →Physician-level review when diagnosis, criteria, treatment intensity, clinical trajectory, and disposition do not point to an obvious determination.
Physician-Led Medical Necessity Review Services →A defensible hospital review separates the question asked at admission from the question asked later in the stay. An appropriate initial inpatient admission does not automatically establish acute medical necessity for every subsequent day—and an initially appropriate observation pathway can change when the patient deteriorates or develops a new medically necessary inpatient need.
What was known, what care was reasonably expected, and what status framework applied?
Did severity, treatment intensity, complications, response, or procedures materially change the clinical need?
Does the patient still require acute hospital care, or have the remaining needs become post-acute or non-medical?
Reconstruct each decision using the information appropriate to that point in the clinical course without collapsing prospective and retrospective reasoning.
For applicable Medicare cases, physician review should identify the governing patient-status framework, evaluate the documented clinical circumstances, and keep procedure classification, admission expectation, and continued-stay necessity analytically distinct.
The physician advisor interprets the medical record through the applicable coverage and patient-status framework, clarifies material clinical facts, supports escalation and dispute workflows, and documents a clear determination. The treating team remains responsible for the patient's clinical care.
Focused physician reassessment when the status or medical-necessity question remains clinically ambiguous.
Concise reasoning that connects material clinical facts to the governing framework and determination.
Flexible physician-review support for temporary volume pressure, vacancies, concentrated queues, or defined initiatives.
Structured review support for selected hospitals, populations, service lines, review categories, or escalation pathways.
Review de-identified examples involving admission status, Two-Midnight analysis, procedure status, continued stay, discharge delay, appeals, post-acute transition, and changing clinical circumstances.
Clinical judgment at the point where status, medical necessity, and hospital operations intersect.
Discuss your hospital case mix, review workflows, physician-advisor capacity, turnaround expectations, escalation pathways, and operational requirements.
Independent physician analysis of disputed status and medical-necessity determinations, including the complete clinical trajectory and applicable coverage framework.
Physician assessment of whether a reported diagnosis or clinical condition is supported by objective clinical indicators and the overall medical record. This service is distinct from a coding-only audit.
Case-based physician education and calibration to improve consistency in status, medical-necessity, documentation, and rationale development.
