CMS Procedure Status & Inpatient-Only Considerations

Current CMS procedure status is incorporated into procedural admission and patient-status review.

Why procedure status matters

Medicare hospital payment rules distinguish services based on current OPPS and inpatient payment requirements. For procedural admissions, the physician reviewer should verify the current CMS status of the procedure before applying a general inpatient-versus-outpatient analysis.

Elective procedure

For an elective surgery, the reviewer identifies the procedure and checks current CMS status. If the service is not subject to an inpatient-only requirement, the fact that surgery occurred does not by itself establish inpatient medical necessity. Clinical complexity, complications, recovery needs and expected hospital course remain relevant.

Unexpected postoperative complication

A procedure typically managed without prolonged hospitalization is followed by major bleeding, respiratory compromise or another serious complication. The appropriate level of care should be reassessed using the patient's actual condition and applicable CMS requirements.
